How to upload a file/copy of an insurance card or medical report into the electronic intake forms?
The instructions might vary slightly depending on what device and updates you have installed but are generally similar to the video here. In verbal summary-- our best recommendation is to use either a mobile phone or tablet to access the portal and then as you proceed through the intake forms, when instructed to do so, use the device's camera to take photos of the items it might ask (like a driver's license, or the completed bladder & bowel diary if a pelvic health client). Or, you can use the phone/tablet and have already taken the photo ahead of time and then just select the photo from the device's camera roll instead of taking live like in this video.
I'm having password & access issues
Troubleshooting Password Issues::
a) The biggest confusion point that we see are when someone is using a password manager and accidentally mixes up their own personal login password with the "two step verification code" which is a one-time use number that is sent to their text messaging. The "two step verification code" gets sent after you enter your password and changes each time you login (unless you have clicked "Trust this Device" on your own personal device). The password only changes if you do a password reset request under the "Forgot Password" option on the login page. If you enter the password incorrectly 5 times, it will block you, notifies us and we will correct it for you within 12-24 hours.
b) The other most common issue is that when using a password manager, someone might have accidentaly saved both their password and the two step verification both as entries for our website. So when they click on the saved password, the don't realize there are 2 options in there and it's not working with the two-step verifiation that accidentally got saved. You can go into the app for your password management and search "ponderarehab.intakeq" and if there's 2 entries there...one needs to be deleted and only save the one that is your correct password.
How do I complete the initial portal registration?
The above basic video shows the portal account registration process. The verbal summary: Once you are ready to do the above process, please click on the APPOINTMENTS tab above & register as a new client. Enter the information asked for and submit. You will recieve a text message with a verification code (one time use). Enter that onto the screen where you have been logging into the portal (your phone/tablet or a desktop/laptop). You now have access to your Pondera Rehabilitation & Fitness secure portal to be able to access the messaging thread, fill out electronic intake forms or self- book visits once cleared to do so.
